New Patients


Patient Registration:

For your convenience, you can register online via our patient portal or you may download the registration forms to complete and bring with you. Please remember to bring your insurance card if you are covered by insurance.

Billing And Insurance:

As a courtesy to our patients, we will file with your insurance company. Please bring your most recent insurance card to every appointment. Any co-payment or deductibles will need to be paid at the time of service. For your convenience, we accept debit and credit cards including Mastercard and Visa.

Appointment Request:

Patients are seen by appointment only, however we try to allow time in our schedules to accommodate patients with urgent problems.

Refill Request:

Our policy is to only refill medications Monday-Friday during regular office hours. We will not refill prescriptions for narcotics (pain pills) or any controlled substance after hours or on weekends. If you call the office for a medication refill, please have the name of the medication and your pharmacy number ready. You can also renew your prescription online at www.spectrumhealth.org, then click on my Spectrum then prescription renewal.

Medical Records Release And Confidentiality:

Your medical information is kept in strict confidence. Before your records are released to your insurance company or any other medical provider involved in your care, you will need to sign a medical release authorization.
